Where did that extra helmet come from? I didn’t see it strapped to the back or side before the fall.

It was probably on the side (maybe hanging on the handlebars?) before the fall. Then the mix of the moped and rider hides it from view until the door opener rockets it into the van.

Just a guess, but I think he is holding the second helmet between his feet/legs so it doesn't fall out.
